Geek Speak: A New Way to Troubleshoot VoIP | thwack

If you?ve deployed VoIP at any level then it is quite likely that you have received complaints about poor call quality.? How exactly does one measure VoIP call quality and how does one go about troubleshooting the cause of the poor quality?

VoIP Metrics

Maintaining high quality VoIP calls can be difficult as VoIP is more sensitive to network delays and packet loss compared to any other network applications.? VoIP quality is measured based on the following metrics:

  • Network Jitter and Delay - excess jitter and delay result in calls breaking up and can be mitigated by the use of jitter buffers, however, too much jitter buffer can cause unacceptable voice delays
  • Packet Data Loss ? packet loss can occur for a variety of reasons including link failure, high congestion levels, misrouted packets, buffer overflows and a number of other factors.? Packet loss can be controlled using packet loss concealment techniques within the playback codec.
  • Latency ? measured in milliseconds (ms) results in voice delay and echo
  • Mean Opinion Score (MOS ) ? indicates the percieved quality of the call and is expressed as a number in the range of 1 (poor) to 5 (excellent)

Troubleshooting the Old Way

Historically, network or VoIP engineers have used a variety of tools to monitor and measure the call quality components including:

  • Passive network monitoring tools that monitor VoIP performance based on network performance statistics and estimate MOS score
  • Protocol analyzers - hardware or software tools that capture and analyze VoIP traffic packets and calculate jitter, and latency directly from the packet stream.
  • Dedicated VoIP tools - originally developed for the telecomm industry and are great for testing IP phone and gateway designs but not as good at solving deployment problems within the network.

Elon Musk pondering Jetsons-like people mover

Entrepreneur Elon Musk of PayPal, Tesla Motors and SpaceX fame has his eyes on an Earth-based people mover that's twice as fast as an airplane and as convenient as riding a train, according to news reports.

While details on the Hyperloop system are scant, Musk recently told PandoDaily?s Sarah Lacey that it is ?something that can never crash, is immune to weather, it goes 3 or 4 times faster than the bullet train?? it goes an average speed of twice what an aircraft would do.?

What?s more, it could be powered by solar panels that generate enough energy to run the system 24 hours a day?? that is, excess energy generated during the day would run the system at night.

Asked by Lacey if the system resembled people movers on the futuristic cartoon the Jetsons, Musk replied, ?Yes.?

Musk is expected to reveal more details on the system soon. He said he may patent the idea and then "open-source the patent to anyone who can make a credible case that they can do it."

The design is thought to be similar the Aeromovel, an elevated train system that is powered by air using a technology that updates the sailboat for the 21st century, notes Clean Technica.

While lack of details, for the moment, on the Hyperloop leave us scratching our heads in incredulous wonder (LA to San Francisco in 30 minutes?), Musk does have a track record that makes this worth paying attention to.?

After all, SpaceX's Dragon capsule successfully delivered cargo to?the International Space Station and may soon be ferrying people there, too. A fast people mover on Earth doesn?t seem so far out of reach.

US drone attack kills at least 5 in Pakistan

U.S. drones fired six missiles and pounded a house and car in the Mir Ali subdivision of Pakistan's North Waziristan tribal region on Sunday.

Local villagers said the drone?targeted a house where militants?reportedly resided.

The villagers said a car has also been damaged in the attack.


Pakistani security officials based in the area said five U.S. spy planes were seen flying over the area during the attack.

A security official said five people had been confirmed dead and four others were injured in the attack.

There was no immediate information about the identity of the victims.

Local residents however said besides Pakistani militants, members of al-Qaida and Afghan Taliban were also hiding in the area.

It was the second attack by U.S. drones in North Waziristan during the holy month of Ramadan.

Fourteen people, all believed to be militants, were killed in a previous drone attack at Dre Nishtar village of Shawal Valley of North Waziristan.

Pakistan officials have?for?an end to U.S.?drone attacks.

Ambassador Sherry Rehman, speaking to the Aspen Security Forum?on Friday,?said the drone attacks have already succeeded in damaging al-Qaida but are now only serving to recruit new militants.

"I am not saying drones have not assisted in the war against terror, but they have diminishing rate of returns," Rehman said, speaking by video teleconference from Washington.

"We will seek an end to drone strikes and there will be no compromise on that," she added.

Pakistan's spy chief, Lt. Gen. Zaheerul Islam, is expected to reiterate the demand in his first meeting with CIA Director David Petraeus, at CIA headquarters in Virginia, next week.

Colorado theater gunman charged with 24 counts of murder

Prosecutors on Monday charged James Holmes with two dozen counts of first-degree murder and more than 100 other violent offenses related to the recent deadly rampage at a Colorado movie theater.

Holmes, who appeared in court with the same cartoonish orange-red hair he had at the time of the shooting, said only one word during Monday's hearing.

"Yes," he answered when asked by the judge if he waived his right to a preliminary hearing within 35 days.

Police say Holmes, 24, blasted his way through a packed movie house during a premiere showing of Batman "The Dark Knight Rises" in Aurora.

Twelve victims died in the attack, 58 others were wounded. The melee is among the worst mass shootings in modern-day American history. Prosecutors filed a total of 142 criminal charges against him, including 116 counts of attempted murder. The 24 murder counts reflect dual charges (premeditated and without remorse) by the prosecutors.

During Monday's hearing, Judge William Sylvester carefully explained to Holmes that the charges carry a minimum sentence of life in prison and a maximum of death. The district attorney near Denver has not announced if she will seek the death penalty against the alleged gunman.

Half of the 110-seat courtroom was filled with victims or their families. One survivor entered the courtroom in a wheelchair with a bandage on her left leg. Blood from a wound could be seen through the bandage. Some stared intently at Holmes, who sat at the defense table to the right of his two attorneys, throughout the 50-minute hearing. Others focused on Judge Sylvester and attorneys as they discussed procedural issues and hearing dates.

Ashley Moser lost her 6-year-old daughter in the attack and was also paralyzed in the shooting. Her aunt, MaryEllen Hensen, was in court on Monday and said the suspect looked more alert in today's hearing. In his first court appearance, Holmes was described by many as being dazed and groggy.

"When we'd seen him before, he looked kind of spaced out or out of touch," Hensen said. "He looked very alert today and very lucid."

Still, she said, "he had an expression and persona of evilness to him."

Some in the court's gallery wore sunglasses. One woman cried openly. Three people, including a woman with a bandaged arm, wore Batman T-shirts.

"It saddens our family that there is a man that has so much hate and evil in his heart," the family of Gordon Cowden said in a statement after Monday's hearing. "We feel confident that our judicial system, as will God and the public, see to it that this evil man receives the punishment he deserves and our hope and prayers are that one day, he will face God with remorse and tremendous sorrow for his actions."

Cowden, 51, had taken his two teenagers to see the midnight premiere. His children escaped the shooting spree without injury.

"Gordon was a man, working with their mom, to raise four children, to build a solid foundation for each; teaching them to love life, love themselves, love others and to love God," his family said. "And though this evil man has caused our family so much hurt and pain he cannot, nor can anyone ever take away or ever permanently damage this foundation of love for each other, for others, for life and for God."

Hensen, who lost a great niece, said she fears sending Holmes to death row will prolong the time it takes for victims to heal.

"As far as the death penalty goes, I am a Christian and I do believe he should just be probably locked away and live with what he did every day of his life," she said.

Holmes was clad in tactical gear and possessed four guns and a stockpile of ammo when he surrendered to officers behind the cinema shortly after the July 20 shooting spree. He did not resist arrest, but investigators have since described the former neuroscience doctoral student as uncooperative.

Police say Holmes legally purchased the guns in May and June but allegedly began stockpiling ammo and other gear four months ago.

"This is not a whodunit. ... The only possible defense is insanity," Craig Silverman, a former chief deputy district attorney in Denver, told the Associated Press.

At Monday's hearing, attorneys also discussed a package the former grad student allegedly sent to his psychiatrist at the University of Colorado Denver.

Authorities seized the package July 23, three days after the shooting, after finding it in the mail room of the medical campus where Holmes studied. Several media outlets reported that it contained a notebook with descriptions of an attack, but Arapahoe County District Attorney Carol Chambers said in court papers that the parcel hadn't been opened by the time the "inaccurate" news reports appeared.

On Friday, court papers revealed that Holmes was seeing a psychiatrist at the university. But they did not say how long he was seeing Dr. Lynne Fenton and if it was for a mental illness or another problem.

The University of Colorado's website identified Fenton as the medical director of the school's Student Mental Health Services. An online resume listed schizophrenia as one of her research interests and stated that she sees 10 to 15 graduate students a week for medication and psychotherapy, as well as 5 to 10 patients in her general practice as a psychiatrist, the Associated Press reported.

Under Colorado law, defendants are not legally liable for their acts if their minds are so "diseased" that they cannot distinguish between right and wrong. However, the law warns that "care should be taken not to confuse such mental disease or defect with moral obliquity, mental depravity, or passion growing out of anger, revenge, hatred, or other motives, and kindred evil conditions."

Experts say there are two levels of insanity defenses. Holmes' public defenders could argue he is not mentally competent to stand trial, like Jared Loughner, who killed six people when he shot Arizona Rep. Gabrielle Giffords in Tucson in 2011. Loughner has pleaded not guilty to charges in the shooting. He has been diagnosed with schizophrenia and is undergoing treatment at a Missouri prison facility in a bid to make him mentally fit to stand trial.

If Holmes' attorneys cannot convince the court that he is mentally incompetent, and he is convicted, they can try to stave off a possible death penalty by arguing he is mentally ill.

Sam Kamin, a law professor at the University of Denver, said there is "pronounced" evidence that the attack was premeditated, which would seem to make an insanity defense difficult. "But," he told the Associated Press, "the things that we don't know are what this case is going to hinge on, and that's his mental state."

Sanctions ultimatum as pressure mounts on Assad

Pressure mounted on Syrian strongman Bashar al-Assad as Western powers drew up a 10-day sanctions ultimatum and a first senior diplomat, his ambassador in Baghdad, defected.

At the United Nations, Britain, France, Germany and the United States submitted a draft text that would give Assad 10 days to implement UN-Arab League envoy Kofi Annan's ceasefire plan or face tough new sanctions.

If Security Council members, including a reluctant Russia, approve it, the resolution would allow for non-military sanctions under Chapter VII of the UN charter if Syrian government forces keep up their offensive on cities.

The resolution condemns "the Syrian authorities' increasing use of heavy weapons, including indiscriminate shelling from tanks and helicopters."

Negotiations on the Western draft and a rival Russian resolution, which does not mention sanctions, are to start on Thursday in New York. A vote must be held before July 20, when the mandate of the UN observer mission in Syria ends.

The draft calls for an "immediate" end to violence by government and opposition forces and demands that Assad's troops return to barracks in line with the Annan plan and UN resolutions passed in April.

If Assad has not "fully complied" within 10 days of the vote, the council "shall impose immediately measures under Article 41 of the UN Charter." Article 41 allows for economic and diplomatic sanctions but not military measures.

The resolution would renew the mandate of the UN Supervision Mission in Syria for 45 days, and calls on the mission to take on more political duties, moving away from monitoring a non-existent ceasefire.

The contingent of nearly 300 unarmed observers should be cut to "the minimum military observer capacity," the resolution says. According to diplomats, the UN leadership wants the observer numbers cut to 150.

Russia opposes the use of Chapter VII but its deputy UN ambassador, Igor Pankin, did not threaten a veto, insisting that negotiations have not even started over the rival texts.

"It is not a time for games, who vetoes first," Pankin said.

"It is a time for deciding on the future of Syria with Syrian plans and people and letting them engage. So I don't think we should be asking questions whether we veto it or not."

Earlier, Annan said the motion should include "clear consequences" for the regime if it fails to act, and reported that even Syria's staunch ally Iran and nervous neighbor Iraq now "support the idea of a political transition."

The latest moves to end the 16-month-old conflict came as the regime continued to crumble from within, with Assad's ambassador to Iraq joining a small but growing list of officials that have defected to the opposition.

An Arab diplomat in Baghdad told AFP that the envoy, Nawaf Fares, had submitted a letter to the Iraqi foreign ministry and would meet officials on Thursday to discuss his travel to another country.

Syria has been gripped by a vicious civil conflict since March last year, when the regime attempted to brutally suppress a pro-democracy revolt and triggered a broader uprising by armed insurgent groups.

Western powers and Syria's Arab and Turkish neighbors have called for Assad to stand down and allow a negotiated political settlement, but Damascus' ally Russia and thus far Iran have stood by him.

The regime and the opposition publicly accept Annan's plan, which has seen unarmed observers deployed to monitor a theoretical ceasefire, but fighting has raged on and rights monitors estimate that 17,000 Syrians have died.

"The Council is now discussing what the next steps should be and what action they can take," Annan told journalists, after privately briefing the 15-member UN Security Council by videophone from Geneva.

"We should hear something in the next few days."

Annan was just back from a tour of the region in which he met Assad and Iranian and Iraqi leaders. He said he and Assad had discussed the nomination of an interlocutor to negotiate with the opposition on behalf of the regime.

"In all frankness, yes we discussed it. He did offer a name. I indicated that I wanted to know a bit about the individual," Annan said, without giving any further details.

Russia's veto of two previous resolutions has angered Syrian opposition leaders, who visited Moscow this week to try to persuade the Kremlin to drop its support for Assad's regime, Moscow's key Middle East ally.

Violence across Syria killed 52 people on Wednesday, 23 of them civilians, 18 soldiers and 11 rebels, according to a Britain-based rights watchdog, which added that 82 people had died the previous day.

It also reported an attack on a bus transporting troops in the northwestern province of Idlib that left as many as 11 dead.

Analysis: Fallout from JPMorgan loss may have just begun

NEW YORK (Reuters) - Jamie Dimon will do his best to put the "London Whale" trading flap behind him on Friday when JPMorgan Chase & Co reports earnings, telling Wall Street that the bank has capped losses from the bad trades and found the key risk management flaw behind the positions.

But that doesn't mean the firm is off the hot seat.

Former employees and experts outside the bank say JPMorgan may be underplaying deeper management problems. Senior executives at the bank missed multiple red flags at the group responsible for the bad trades, including high turnover among risk managers, that raise questions about how far up the chain blame should be assigned.

"One would think issues would have been escalated internally," said Leslie Rahl, managing partner of Capital Market Risk Advisors and a former derivatives head at what is now Citigroup. "If there's not more disclosure about the governance on Friday, they still have questions that need to be answered."

JPMorgan declined to comment on any aspect of ongoing investigations or on responsibility for the losses.

Dimon and his team on Friday plan to disclose more about the bad derivatives bets its Chief Investment Office made on portfolios of corporate bonds, bets likely to cost the bank $4 billion to $6 billion, according to a source. The update will come as the bank reports second-quarter results that Chief Executive Jamie Dimon said were on the whole "solidly profitable." [ID:nL2E8HS71S]

Dimon has moved quickly to distance himself from the losses and determine who to blame. As part of an internal investigation, bank executives were asked to draw up lists of risk managers and other executives who bear responsibility, a source said.

JPMorgan plans to attribute the losses in large part to a flaw in the way it tracked and limited risk-taking in the CIO, the source said. Instead of limiting risks for every individual trader as is standard on Wall Street, the bank assigned limits to groups of traders, allowing individuals to take outsized positions.

Ina Drew, who ran the Chief Investment Office and was deeply trusted by Dimon, has resigned, and a number of her lieutenants have had their trading terminals revoked, sources said.

"A RIGHT TO RELY ON THEM"

Dimon testified to Congress last month that "complacency" following years of success at the CIO led him and others to miss signals about the office's money-losing trades. "This synthetic credit portfolio did earn several billion dollars of income in the three or four years before it just lost some of it," Dimon told the Senate Banking Committee on June 13.

In addition to blaming Drew for the surprise loss, Dimon went on to criticize his chief financial officer, the bank's risk managers and others in the chain of command for failing to alert him to the risk that was accumulating.

"I have a right to rely on them," he said.

Barry Zubrow, who was chief risk officer from late 2007 until this January, the period when the CIO ramped up its risk appetite in London, appears to be most vulnerable to criticism, said people inside and outside JPMorgan.

Another problem for Zubrow: his brother-in-law Irvin Goldman was top risk manager in the CIO from February until he was replaced in May. Zubrow, who now heads corporate and regulatory affairs, did not return a call for comment.

The bank's chief financial officer is Douglas Braunstein. A long-time investment banker with an expertise in mergers and acquisitions, not in trading, Braunstein has been the CFO since mid-2010.

Braunstein is scheduled to give his usual presentation when the bank posts earnings on Friday, signaling to outsiders that Dimon does not assign much blame for the losses to his CFO.

Whatever Dimon says on Friday, a host of international regulators and agencies are probing the trading mishap, including the U.S. Securities and Exchange Commission, the UK's Financial Services Authority, the FBI, the Federal Deposit Insurance Corp., the U.S. Commodity Futures Trading Commission, the U.S. Treasury's Office for the Comptroller of the Currency, and the Federal Reserve Board and Federal Reserve Bank of New York.

AN UNUSUAL STRUCTURE

Dimon established the CIO in 2005 and asked Drew to manage it. The office, which now oversees about $350 billion of cash, has an unusual mandate: it is supposed to hedge the entire bank's risk, but can also take big bets to goose profits. Few banks allow the same group to do both.

The group earned billions of dollars during the financial crisis, but there were signs of trouble. In the past six years, there have been at least five different heads of market risk management within the CIO.

"It probably was a management mistake that no one asked why people were leaving," said a former senior risk manager at the bank.

One reason for departures may have been that pay was relatively low, at least by Wall Street standards, according to two people familiar with the group. The head of market risk within a business line at JPMorgan receives $1 million to $1.5 million of annual compensation, while traders who they monitor can make multiples of that amount. (Drew was awarded $31.5 million over 2010 and 2011, but the bank is likely to try to take at least some portion of that money back.)

The risk officers at the CIO also may have felt dual loyalties, sources said. The chief executives of the business lines at JP Morgan work side-by-side with the chief risk officer in picking their departments' risk managers, several sources said. That may create conflicts in a position that demands independence.

New executives appointed in the past few weeks, including Drew's replacement, Matt Zames, have moved quickly to build a new risk management structure for the CIO and the corporate bank. Dimon, in congressional testimony on the loss, called Zames a "world class risk manager."

"When we make mistakes, we take them seriously and often are our own toughest critic," he testified last month.

Another key question for most outside observers was why the bank avoided standard Wall Street practice by allowing traders to take massive positions. One trader in the CIO took big enough positions in credit derivatives markets to earn the nickname "London Whale."

"Limit structures are usually written down, and traders come in every morning knowing how much they are allowed to buy," said one senior banker with trading and risk-management experience at JPMorgan, who asked for anonymity. "If you can drive four Mack trucks through it sideways, I don't think that's a limit structure."

Incentives for traders also are worth investigating, said one former risk manager. In Dimon's congressional testimony, he said traders in the CIO were not paid on a formula that encouraged profits over risk reduction.

A former executive, however, said traders in the CIO had incentives to swing for the fences, at least when he was in that office. Their bonuses were determined in part by their ability to outperform an internally created index of the return on a standard bank treasury portfolio. The executive said he did not know if the index was still used, and bank spokesmen declined to comment.

A day after winning European Union approval for a huge bank bailout and breathing space on its deficit program, Prime Minister Mariano Rajoy warned Parliament that Spain's future was at stake as it grapples with recession, a bloated deficit and investor wariness of its sovereign debt.

"We are living in a crucial moment which will determine our future and that of our families, that of our youths, of our welfare state," Rajoy said to catcalls from the opposition socialists and other parties as he revealed the biggest single amount of projected deficit savings in modern Spanish history.

He spoke as thousands of miners stung by a huge cut in government subsidies marched through downtown Madrid and clashed with riot police outside the Industry Ministry.

The spending cuts, designed to cut ?65 billion off state budgets by 2015, include a wage cut for civil servants and members of the national parliament and a new wave of closures at state-owned companies. Spain will also speed up a gradual increase in the retirement age from 65 to 67. They are to be approved officially Friday at a Cabinet meeting.

Spain has had to digest round after round of austerity measures since Rajoy's conservative government took power in December. Until now, there have been ?60 billion ($73.71 billion) in spending cuts and tax hikes by the central government or regional administrations. If you include measures taken by the previous, Socialist government, the number goes up to ?75 billion. Now, albeit spread over two-and-a-half years, comes another ?65 billion.

"This is the reality. There is no other, and we have to get out of this hole and we have to do it as soon as possible and there is no room for fantasies or off-the cuff improvisations because there is no choice," Rajoy told Parliament.

The measures are in exchange for the bank bailout of up to ?100 billion ($122.85 billion) granted to Spain by the other 16 countries that use the euro and extra time to cut the Spanish budget deficit. Finance ministers approved the bailout program at meetings in Brussels this week and as much as ?30 billion could flow to Spain's banks by the end of the month. The country's banks are saddled with billions of euros in toxic loans and assets following the collapse of the country's real estate market. The full amount Spain will seek is not yet known.

Europe's finance ministers also this week extended Spain's deadline for achieving a budget deficit of less than 3 percent of its annual economic output, until 2014. The size of Spain's economy in 2011 is estimated to have been $1.5 trillion.

The bank aid and the deficit-cutting come at the cost of greater EU supervision of Spain's finances, both for the government and the banks, even though Rajoy's government insists it has given up no sovereignty.

"In exchange for the bank bailout agreement, Brussels, the ECB and the IMF have placed Spain and its institutions in a situation of strict monitoring and control," Spanish newspaper El Pais said in an editorial Wednesday.

The concern among investors and Europe-watchers is that further austerity cuts will push Spain's economy further into recession in the short term, making it even harder for the government to trim its deficit.

Spain ? the fourth-largest economy in the eurozone ? has been struggling to keep a lid on its government deficit in the midst of a recession while trying to support its troubled banking industry. There are fears that should Spain need a bailout of its own as did Greece, Ireland and Portugal, the eurozone would struggle to finance it, pushing the region further into recession.

However, said Mark Miller of Capital Economics in London, markets are concentrating on the planned bank bailout, which could get the economy up and running again by providing businesses and households.

So if the bailout works, it should eventually compensate for the bitter cocktail of the new austerity package. "'It is the classic case of short-term pain for longer-term gain," Miller said.

Wednesday's increases in sales tax include a 3 percentage point hike on products and services like clothing, cars, cigarettes and telephone services to 21 percent and a 2 percentage point increase on goods such as public transport fares, processed foods and bar and hotel services to 10 percent. The sales tax on basic goods like bread, medicine and books stays at 4 percent.

Other measures outlined Wednesday included:

??660 million of cuts in government spending beyond the reductions already outlined in the 2012 budget

? wage cuts for civil servants and members of the national parliament

? further closures of state-owned companies

? tax deductions for homeowners to be scrapped

? a 30 percent cut in the number of town councilors

? slight reduction in unemployment pay, designed to encourage jobless people to seek work more quickly.

? 20 percent cut in government subsidies to political parties and labor unions.

? possible privatization of ports, railways and airports

In Brussels, spokesman Simon O'Connor said the European Commission, the EU's executive arm, welcomed the government's announcement of the new measures as "an important step to ensure that the fiscal targets for this year can be met."

Financial markets greeted news of the measures. The interest rate on Spain's benchmark 10-year bond dropped 0.22 percent to 6.57 percent, away from the 7 percent levels it reached earlier this week, while the country's IBEX stock index was up by 1.1 percent in afternoon trading.

Howard Stern & Howie Mandel Argument Gets Heated On AGT (Video)

Things got heated last night on America?s Got Talent between Howard Stern and Howie Mandel. There is no love lost between those two that is for sure. 70 ?year old Big Barry was at the center of the tense argument between Howie and Howard on AGT. The lounge singer has made it through two rounds of auditions largely due to the support from Mandel. On the flip-side of that is Stern who is not at all a supporter of Big Barry. Well the differences between the two men on this subject took a turn for the worse last night. After the lounge act singer got done doing his thing, which I have to say wasn?t as great as Howie said but didn?t suck as much as Howard made it seem either, Mandel began giving Big Barry props for his performance. Stern was not having it anymore and lets just say the two men had some serious words while Sharon Osbourne just sat thee very uncomfortable between the two feuding men. Philadelphia, July 10, 2012 -- The American College of Physicians (ACP) and the Alliance for Academic Internal Medicine (AAIM) today unveiled a high value, cost-conscious care curriculum to help train internal medicine residents about how to avoid overuse and misuse of tests and treatments that do not improve outcomes and may cause harms.

The free curriculum, available at www.highvaluecarecurriculum.org, is designed to engage internal medicine residents and faculty in small group activities organized around actual patient cases that require careful analysis of the benefits, harms, costs, and use of evidence-based, shared decision making. The flexible curriculum consists of ten, one hour interactive sessions that can be incorporated into the existing conference structure of a program.

"Physicians receive little specific training about identifying and eliminating wasteful diagnostic and treatment options," said Cynthia D. Smith, MD, FACP, ACP's Senior Medical Associate for Content Development and the lead author of "Teaching High-Value Cost-Conscious Care to Residents: The AAIM-ACP Curriculum," published online in Annals of Internal Medicine, ACP's flagship journal. "Residency training is an excellent time to introduce the concept of high value, cost-conscious care because the habits that residents learn during training have been shown to stay with them throughout their professional careers."

Health care expenditures are projected to reach almost 20 percent of the United States' GDP by 2020. Many economists consider this spending rate unsustainable. Up to 30 percent, or $765 billion, of health care costs were identified as potentially avoidable -- with many of these costs attributed to unnecessary services.

The committee that developed the curriculum consisted of ACP-AAIM members and staff, program directors, associate program directors, a department chair, residency faculty, and internal medicine residents.

What is High Value, Cost-Conscious Care?

High value, cost-conscious care means that a test or treatment has benefits that make its potential harms and costs worthwhile. Some expensive tests and treatments have high value because they provide high benefit and low harm. Conversely, some inexpensive tests or treatments have low value because they do not provide enough benefit to justify even their low costs and may even be harmful.

Because misuse and overuse of medical interventions that do not improve patient health contribute significantly to the unsustainable growth of health care spending, ACP launched a High Value, Cost-Conscious Care initiative in 2010 to help physicians and patients understand the benefits, harms, and costs of tests and treatment options for common clinical issues.
